The aims of this study were to describe the population pharmacokinetics of phenytoin after a single oral dose of its sustained release (SR) formulation (Zonalon) and to evaluate the potential influence of demographic characteristics, concomitant drug use and comedication on phenytoin pharmacokinetics. A total of 200 plasma concentrations of phenytoin obtained after the administration of one dose of the SR formulation were analysed by a nonlinear mixed effect model. The following covariates were tested for an influence on the population pharmacokinetics of phenytoin: sex, age, body weight, height, body mass index, creatinine clearance, albumin and total protein levels, use of aminophylline and other antiepileptic drugs, cardiovascular disease, diabetes, lipid disorder, hypercholesterolemia, hypertension, smoking habit, alcohol intake, comedication with other drugs (tramadol, cimetidine and acetylsalicylic acid) and comedication with caffeine, theophylline and paracetamol. Phenytoin clearance and volume of distribution were significantly influenced by the age of the patient (a = 0.0062 and 0.0916 respectively), creatinine clearance (a = 0.0441 and 0.0292) and the use of other antiepileptic drugs (a = 0.0269 and 0.0457, respectively). The other covariates did not have a significant influence on the population pharmacokinetics of phenytoin. The population pharmacokinetics of phenytoin after one oral dose of the SR formulation is similar to those of phenytoin after multiple doses of the immediate release formulation.
